Recurrent Streptococcus bovis meningitis in Strongyloides stercoralis hyperinfection after kidney transplantation: the dilemma in a non-endemic area.

چکیده
Post transplant parasitic infections are a rarity and occur in around 2% of transplant recipients; the intestinal helminth Strongyloides stercoralis (Ss) is found in contaminated soil in hot and humid tropical and subtropical regions of Africa, South and East Asia, and South America. Infective larvae from contaminated soil enter the host venous system from the skin to end up in the lungs and are then ingested into the GI tract by the pharynx where they mature into adults. Approximately half of the immune competent hosts become carriers and the remainder usually present with mild abdominal and respiratory symptoms. Transplant recipients receive steroids, have altered cellular immunity, and are at increased risk for hyperinfection syndrome because of an accelerated auto-infective cycle, rapid multiplication, and systemic larval dissemination. This can result in serious complications including GI bleeding, bacteremia, meningitis, liver abscesses, pneumonia, and death. We report a Strongyloides hyperinfection syndrome in a Saudi kidney recipient with recurrent Streptococcus bovis bacterial meningitis and significant weight loss 2 months after deceased donor kidney transplantation.

Pulmonary strongyloidiasis following renal transplantation without travel to an endemic area
We report a case of Strongyloides stercoralis hyperinfection in an immunosuppressed individual occurring in a non-endemic area. Geographic risk is not sufficient to rule out Strongyloidiasis in susceptible individuals presenting with severe pulmonary disease.
متن کاملStrongyloides stercoralis hyperinfection transmitted by liver allograft in a transplant recipient.
We describe a case of Strongyloides stercoralis hyperinfection in a liver allograft recipient 2.5 months after transplantation. The patient lives in Spain, which is not considered an endemic country for strongyloidiasis, and denied prior residence or travel to any known endemic area. The initial symptoms were fever and vomiting, and he subsequently developed a severe respiratory disease. An end...
متن کاملStrongyloides stercoralis hyperinfection presenting with symptoms mimicking acute exacerbation of chronic obstructive pulmonary disease.
Hyperinfection syndrome with Strongyloides stercoralis is not uncommon in immunocompromised patients. We present 2 fatal cases of Strongyloides hyperinfection with initial presentation mimicking acute exacerbation of chronic obstructive pulmonary disease (COPD). Both cases had a history of COPD and had received systemic steroid treatment before or during admission. The initial chest radiograph ...
متن کاملHIV infection associated with Strongyloides stercoralis colitis resulting in Streptococcus bovis bacteraemia and meningitis.
We report the case of an HIV infected patient with Streptococcus bovis bacteraemia and meningitis associated with gastrointestinal Strongyloides stercoralis infection. To our knowledge, this has been reported once previously and serves as a reminder to actively exclude asymptomatic S stercoralis infection in HIV infected individuals presenting with bacteraemia.
متن کاملStrongyloides Hyperinfection Syndrome Combined with Cytomegalovirus Infection
The mortality in Strongyloides hyperinfection syndrome (SHS) is alarmingly high. This is particularly common in bone marrow, renal, and other solid organ transplant (SOT) patients, where figures may reach up to 50-85%. Immunosuppressives, principally corticosteroids, are the primary triggering factor. In general, the clinical features of Strongyloides stercoralis hyperinfection are nonspecific;...
